Chronic Disease Management
Chronic Disease Management
A disease or condition is considered chronic if it lasts for one year or more. Managing chronic disease may include screenings, routine check-ups, monitoring and coordinating treatment, lab tests, and patient education. The goal of chronic disease management are optimized patient care and a supportive provider-patient relationship, which lead to improved health outcomes.
